The Philippine Statistics Authority, or PSA, has revealed that the application for a PSA certificate can be made via a Self-Service Kiosk at select PSA Civil Registry System (CRS) outlets. With this service, you may avail of the following civil registry document services:
  • Copy Issuance of Birth, Marriage, and Death Certificates
  • Certificate of No Marriage Record (CENOMAR)
  • Advisory on Marriages or Certificate of Marriage (CEMAR)
  • Certificate of No Death Record (CENODEATH)
  • Advisory on Deaths

How to request an appointment online with PSA?

To set an appointment with PSA, follow the step below.
Step 1: Go to the official PSA Online Appointment System website (https://appointment.psa.gov.ph/).
Step 2: Click the image with the PSA logo in the center of the screen.
Step 3: Scroll down and check the "I Agree" checkbox.
Step 4: Choose the PSA CRS outlet where you would like to set an appointment. (If you are in Metro Manila and requesting a PSA certificate that underwent Court Decrees and Legal Instruments for the first time, make an appointment at East Avenue.)
Step 5: Enter your name, email address, and mobile number. After that, PSA will send a copy of your appointment sheet through email. You will receive a verification code on your email address.
Step 6: Select the type of PSA certificate that you would like to request. If you are requesting a certificate for someone else, please indicate their names and your relationship with the certificate owner.
Step 7: Choose the date and time you plan to visit your chosen PSA outlet.
Step 8: Remember the important details of the appointment. Verify that your name, email address, and mobile number displayed on the screen are correct. It is important your name exactly matches your ID. Use your complete full name, no initials.
Step 9: You will then receive a copy of your Appointment Slip via email. Save the PDF file on your phone or print it. You'll need to print the entire form, including the QR code.
Step 10: Go to your selected PSA outlet on the date of your appointment. Arrive at the location on time or a few minutes early. Bring your own pen and protect yourself with a face mask.

NOTE: Walk-ins are also still accepted in some CRS outlets. This may be subject to government rules and regulations as announced.

How to apply for a PSA Certificate via the CRS Outlet's Self-Service Kiosk?

Selected PSA CRS Outlets also have Self-Service Kiosks (SSK). Here's how to use the kiosk.
Step 1: Secure a queue ticket number upon entering the PSA CRS Outlet.
Step 2: Proceed to any available self-service kiosk.
Step 3: Encode the required information.
Step 4: The self-service kiosk will print your transaction slip.
Step 5: Select your preferred payment option. You may pay online or proceed to the Payment Area for cash payment. If you choose to pay online, continue by entering your credit or debit card information into the kiosk.
Step 6: When your payment is accepted, the self-service kiosk will then print your electronic OR (Official Receipt). If you choose to pay in cash or through other payment partners, proceed to the Payment Area, submit your printed transaction slip from the self-service kiosk, valid ID, and complete your payment. If you are not the document owner, you will need to present the original and photocopy of the document owner's valid ID, Authorization Letter/Special Power of Attorney (SPA)/Affidavit of Kinship, and original and photocopy of your valid IDs.
Step 7: Specify your preferred payment method and settle your payment to the Collecting Officer, who will then issue an Official Receipt and inform the estimated schedule of release.
Step 8: Please check your Official Receipt and count your change, if any, before leaving the Payment Area.
Step 9: [Document Releasing Phase] Proceed to the Releasing Area on the given date and time.
Step 10: You may claim your document by presenting to the Releasing Officer your Official Receipt, Queue Ticket Number, Application Form, original and photocopy of valid IDs, Authorization Letter, and other supporting documents.

How to apply for a PSA certificate via the CRS outlet without using the Self-Service Kiosk?

Here's how to secure your Civil Registry Documents at the CRS outlets.
Step 1: Once inside the PSA CRS Outlet, get your Queue Ticket Number (QTN). Make sure to get the correct application form for the document you are requesting and that you have all the requirements on hand.
Step 2: Fill out the Application Form. Ensure that all the details are accurate and correct.
Step 3: Proceed to the Payment Area. If you are pregnant, a PWD (Person with Disability), or a senior citizen, proceed to the designated area.
Step 4: Present your Application Form, QTN, and other PSA requirements. If you are the document owner, present your valid IDs. If you are the document owner's representative, present the following: (1) Original and photocopy of the document owner's valid ID, (2) Authorization Letter/Special Power of Attorney (SPA)/Affidavit of Kinship, (3) Original and photocopy of your valid IDs.
Step 5: The Encoder will screen your Application Form and encode your request, while the Collecting Officer will get your payment, issue an Official Receipt, and inform you of the estimated schedule of release.
Step 6: You may pay using cash or through PSA's other payment partners like GCash and Maya.
Step 7: Please check your Official Receipt for the date of release and count your change, if any, before leaving the Payment Area.

Step 9: You may claim your document by presenting to the Releasing Officer your Official Receipt, Queue Ticket Number, Application Form, original and photocopy of valid IDs, Authorization Letter, and other supporting documents. Please check your documents before leaving the Releasing Area.
Step 10: Make sure to carefully review all the entries in your document for any incorrect information. If you have any questions or clarification on the document, you may proceed to the Public Assistance and Complaints Desk (PACD).
